Online Registration for 2025-26 School Year

Students interested in participating in sports at Longfellow must do the following:
- Complete the Online Registration. You may sign up for one sport each season – fall, winter, spring
- Current Sport Physical completed, signed by parent AND doctor. Must be dated AFTER April 1, 2023 this year.

Athletics – Sports Physicals
Your child may participate in the Longfellow Middle School. Longfellow competes in the Three Rivers Conference which is composed of teams from Aquinas, GET, Holmen, LaCrescent, Logan, Lincoln, Longfellow, Onalaska, Sparta, Tomah, Viroqua, West Salem, Westby, and Winona Middle Schools.
Prior to participating in any practice or competition session, a student must turn in a Wisconsin Interscholastic Athletic Association (WIAA) athletic physical card (Click on FORMS link to the left). If your child had a physical examination after April 1, 2024 it will be good for the entire school 2025-26 school year.
It is suggested that you make an appointment for a physical examination as soon as possible during the summer. A link to the physical form is below. Hard copies are also available in the Longfellow office.
