Athletics – Sports Physicals

Your child may participate in the Longfellow Middle School. Longfellow competes in the Three Rivers Conference which is composed of teams from Aquinas, GET, Holmen, LaCrescent, Logan, Lincoln, Longfellow, Onalaska, Sparta, Tomah, Viroqua, West Salem, Westby, and Winona Middle Schools.

Prior to participating in any practice or competition session, a student must turn in a Wisconsin Interscholastic Athletic Association (WIAA) athletic physical card. If your child had a physical examination after April 1, 2023 t is required for the 2024-2025  school year. It is suggested that you make an appointment for a physical examination as soon as possible during the summer. A link to the physical form and alternative year card is under FORMS. Hard copies are also available in the Longfellow office.
